Service Requests
Below is a list of our services you may request online for your convenience. From our family to yours, we thank you for choosing Town Center Family Medicine.
Schedule an Appointment
Online appointments must be made three working
days in advance. Online appointments are most
convenient for well woman, well man, well baby,
annual visits, physicals, etc. Please make sure you give us a phone number and e-mail address where we can confirm the appointment.
Prescription Refill Request
Some prescriptions cannot be refilled without an
office visit. If you have not seen your provider within
the last year, your prescription cannot be filled.
You will need to make an appointment so your
provider can follow up on your health adn side effects of the medicine. Prescription refills will be done only during regular office hours so that we can contact the pharmacist. We will fax or call your prescription within 48 hours.
Specialist Referral Request
If your insurance company requires referrals for you to see a specialist, you must request a referral before making an appointment with the specialist. Please allow us 3 to 5 days to complete the referral. If your provider has not referred you to the specialist, you might need an office visit for the provider to approve the referral.

Update Personal Information
If you have recently moved, changed your phone number, insurance or other personal information, please submit a form for us to update your personal information. If its been longer than 9 months, please download patient registration form and bring with you at your next visit.
Ask a Nurse/ Provider
We are providing this service for your use if you have medical questions, requests that you would leave on the phone system. We know that its sometimes difficult to leave a detailed message over the phone. We are trying to make our services as convenient as possible for you. This mailbox will only be checked several times/day during office hours only. If you have an urgent medical question or a question that is time sensitive, please call our office, press 2 for appointments and our staff will be able to help you. After office hours, if you have an urgent medical problem that cannot wait until the office reopens, please leave a message on the urgent mailbox. A provider will return your call within 30 minutes. Providers are on call 24 hours/day every day. If it is a life threatening problem, please call 911.
Medical Records from us
If you would like us to provide you with your medical records, please fill out form and submitted to us. Please note there is a charge to process this request. If you would like to have our office request medical records from your former physician, please fill out this form, and either give it to your previous physician, or bring it to us, and we can request your chart.
